• Conduct thorough assessments of clients to identify their social, emotional, and environmental challenges, using evidence-based methods.
• Develop personalized service plans that outline specific goals, interventions, and means of monitoring progress toward each goal.
• Provide individual and family counseling, addressing issues such as anxiety, depression, behavioral challenges, and coping strategies.
• Collaborate with clients to formulate actionable service plans and connect them with relevant community resources, such as housing assistance, mental health support services, and educational resources.
• Offer crisis intervention and support, performing risk assessments and safety planning as needed, ensuring client safety and well-being at all times.
• Engage with local community agencies and organizations to coordinate services and advocate for client needs, expanding our collective impact.
• Participate actively in outreach initiatives to raise awareness about our agency’s services, contributing to community engagement and collaboration.
• Attend case conferences, team meetings, and ongoing professional training sessions to foster continuous learning and development.

Your primary focus will be supporting a diverse range of students and their families struggling with various life circumstances, including but not limited to educational challenges, socio-economic hardships, and mental health issues. You will work with this population to enhance their quality of life and educational outcomes through tailored support strategies and resources.

• A Master’s degree in Social Work from an accredited institution is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Proficiency in trauma-informed care, and understanding of social and economic justice issues is essential.
• Exceptional communication skills, cultural competency, and a strong problem-solving mindset are crucial for this role.

• A minimum of one year of experience in a social service agency or similar setting, particularly with school-aged children and families.
• Previous experience with case management and crisis intervention in a school environment.
• Familiarity with community resources available in the Fort Collins area and knowledge of local educational systems is beneficial.
