Key Responsibilities

• Conduct detailed assessments of clients' social, emotional, and environmental needs to identify key areas for intervention.
• Craft personalized service plans that set achievable goals, delineate interventions, and monitor progress closely.
• Provide empathetic counseling to both individuals and families facing a range of significant life transitions, including but not limited to trauma, loss, and mental health challenges.
• Engage with clients in collaborative discussions to enhance their understanding of available community resources and facilitate access to necessary services.
• Implement crisis intervention strategies, including comprehensive risk assessments and safety planning when faced with emergencies.
• Nurture relationships with partner agencies, facilitating coordinated services and advocating effectively on behalf of clients.
• Actively participate in community outreach initiatives aimed at increasing awareness of agency services, addressing client needs, and demonstrating the impact of social work.
• Engage in continuous professional development through participation in case conferences, team meetings, and specialized training.

Patient Population / Client Focus

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW) in Colorado.
• Demonstrated knowledge of best practices in social work, including principles of trauma-informed care, social justice frameworks, and community resource navigation.
• Exceptional communication skills, with a proven ability to perform effectively within a team and build rapport with clients from varied backgrounds.

Preferred Experience

• At least one year of experience in a social service agency or similar environment is highly desirable.
• Experience working with vulnerable populations and addressing multifaceted socio-economic issues.
• Familiarity with case management software and electronic health records systems is advantageous.
