As a remote social worker at Lifeline Community Care, you will have the flexibility to manage your work environment while staying connected with team members and clients through digital communication tools. The role will integrate clinical responsibilities with community outreach, allowing you to directly engage with clients both in virtual consultations and through local community engagements. • Conduct comprehensive assessments of clients experiencing symptoms of major mental illnesses or severe emotional disorders, utilizing evidence-based frameworks.
• Develop, implement, and monitor individualized care plans based on the unique needs and requests of clients.
• Facilitate individual, group, and family therapy sessions, providing focused interventions aimed at relieving distress and enhancing coping strategies.
• Participate actively in team meetings, contributing to collaborative discussions that drive service improvements and quality enhancements for our programs.
• Engage in crisis intervention, providing immediate support and resources to clients in distress, ensuring their safety and well-being.
• Maintain detailed and timely records of client interactions, treatment plans, and progress notes to adhere to organizational compliance and reporting standards.
• Work closely with community resources, service providers, and stakeholders to coordinate care and integrate services for clients.
• Provide educational resources and workshops in the community to promote mental health awareness and engagement.

The Licensed Clinical Social Worker will primarily support a diverse clientele, including individuals and families from various cultural backgrounds in Hempstead and surrounding areas. Many of our clients may be experiencing socioeconomic challenges that contribute to their mental health struggles. Your role will focus on providing culturally competent care that respects and values the individuality of each client, promoting empowerment and choice in their treatment journey.

• A Bachelor’s Degree in Social Work (BSW) or a related field is a requisite. A Master’s Degree in Social Work (MSW) is preferred.
• A valid Licensed Clinical Social Worker (LCSW) credential issued by the appropriate New York State authorities.
• Two or more years of clinical experience within mental health or community health settings.
• Demonstrated expertise in risk assessment, crisis intervention, and the treatment of various mental health conditions.
• Strong interpersonal skills with excellent verbal and written communication abilities.
• Proficiency in managing case files, maintaining confidentiality, and adhering to ethical practices in social work.

• Experience working in remote settings and familiarity with telehealth protocols.
• Engagement in community outreach initiatives and collaboration with non-profit organizations.
• Background in providing trauma-informed care.

The annual salary for this position ranges from USD 70,000 to 85,000, based on experience and qualifications.
