As a remote Social Worker, you will work closely with school-based teams, maintaining a flexible schedule primarily between Monday and Friday from 8:00 AM to 4:30 PM, with potential occasional evening and weekend hours based on client needs. • Perform comprehensive evaluations of seniors' psychosocial requirements, incorporating emotional, mental, and social factors.
• Collaborate with clients, families, and educators to develop tailored care plans that align with specific personal goals and preferences.
• Provide compassionate counseling and emotional support, guiding clients through significant life transitions and challenges.
• Advocate for clients' access to pertinent community resources, ensuring they receive comprehensive support and services tailored to their unique needs.
• Engage with interdisciplinary teams, including educators, caregivers, and healthcare professionals, to share insights and coordinate care holistically.
• Conduct regular check-ins to monitor client progress, assess living conditions, and address any emerging concerns or threats to safety.
• Facilitate effective communication among family members involved in the client's care, helping to resolve conflicts and improve shared decision-making.
• Educate clients and their families on available community resources, support groups, and programs that enhance their quality of life and social connectivity.
• Ensure meticulous documentation of assessments, care plans, progress notes, and other critical client interactions, maintaining compliance with applicable regulations.
• Stay updated on best practices in social work and relevant policies, ensuring adherence to ethical standards.

This role primarily centers around serving seniors and families facing complex emotional and psychological challenges. You will be instrumental in empowering individuals to regain control over their anxiety and depression while equipping them with essential tools for emotional well-being.

• Master's degree in Social Work from an accredited institution.
• Current and valid LCSW licensure in the state of Arizona.
• Minimum of two years of experience in social work, with a preference for experience in educational settings or geriatric care.
• Comprehensive understanding of social work principles, aging challenges, and community resources.
• Exceptional skills in assessment, counseling techniques, and problem resolution.
• Strong ability to empathize, exhibit compassion, and build trust with seniors and their families.
