• Work directly with clients in individual and group sessions, providing therapeutic interventions to those experiencing major mental health challenges.
• Conduct thorough assessments and develop integrated care plans that address immediate client needs while connecting them with necessary social services.
• Regularly monitor client progress, adjusting treatment plans to enhance their mental health outcomes and ensuring adherence to the goals set in collaboration with the client.
• Create a trusting rapport with clients through empathetic engagement, demonstrating a solid understanding of various cultural backgrounds, beliefs, and interpersonal dynamics.
• Participate actively in team meetings and case reviews, contributing to an interdisciplinary approach that ensures comprehensive care delivery.
• Provide crisis intervention as necessary, employing de-escalation techniques and therapeutic modalities to assist clients in acute distress.
• Ensure all documentation is completed timely and accurately, including progress notes, treatment plans, and compliance reports, in alignment with agency requirements.

The LCSW will primarily work with a diverse range of clients who experience severe emotional disorders and major mental illness. This role demands a deep commitment to understanding the complex challenges faced by individuals in underrepresented communities in Fort Collins. • A valid, unrestricted license as a Clinical Social Worker (LCSW) in the state of Colorado.
• A Bachelor’s degree in Social Work or a related field is required, with a preference for those holding a Master’s degree in Social Work.
• At least two years of experience in behavioral health or social services, specifically working with individuals facing mental health challenges.

• Experience working in a community health or outpatient settings.
• Familiarity with evidence-based practices and current therapeutic approaches utilized in mental health treatment.
• Proficiency in electronic health record (EHR) software for documentation and client management.

This role offers a competitive annual salary between USD 70,000 and 85,000, commensurate with experience and qualifications.
