Job description

Humana/iCare is seeking a Care Management Manager to join the team in the Family Care Partnership (FCP) program. You will report to the Associate Director, Care Management Support. You will oversee a team of nurses and human service professionals working in the field who are responsible for care management services through the FCP program. Develop your leadership skills all while doing your part to improve the lives of others.

Responsibilities:

  • Screen, interview, orient and establish dynamic and successful associates.
  • Manage and evaluate the performance of associates assigned to their teams.
  • Promote a collaborative team environment and positive work culture.
  • Provide leadership and feedback to encourage professional development and associate retention.
  • Convene team meetings to the Interdisciplinary Team (IDT) to provide case staffings and share information.
  • Provide coaching to associates to meet member needs.
  • Use established systems to monitor documentation to ensure quality and contract compliance.
  • Communicate with program management about the challenges faced by the IDT and work with management to address these challenges.
  • Work with FCP and iCare/Humana leadership to develop and implement programs to achieve identified metrics such as HEDIS elements and financial performance targets and other strategic projects as identified.
  • Work with other departments to resolve case specific issues with the IDT.
  • Collaborate with the FCP Learning and Development team to provide training to associates, ensuring they stay current with the Family Care Partnership model and other best practices or other program development plans.
  • Collaborate with Program and Company Management to provide leadership to the IDT.

Required Qualifications

  • Bachelor's Degree or Registered Nurse
  • Reside within the State of WI
  • Four (4) or more years of full time, professional experience.
  • Two (2) or more years of people management/supervisory experience.
  • 1 year of experience in managed care, long term care programs, or in a health care field.

Preferred Qualifications

  • Reside within southeastern Wisconsin

  • Workstyle: Remote, work from home
  • Location: Southeast Wisconsin
  • Travel: up to 10% to Humana/iCare's Milwaukee office location

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


$78,400 - $107,800 per year


This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
