Key Responsibilities
  • Conduct comprehensive clinical evaluations to assess the psychosocial needs of patients and families in hospice care.
  • Provide individualized counseling and therapeutic interventions, focusing on empowering clients and enhancing their coping mechanisms.
  • Monitor medication side effects in collaboration with psychiatrists and adapt treatment plans accordingly.
  • Facilitate crisis intervention sessions to address acute emotional and psychological distress.
  • Develop and implement a detailed aftercare plan tailored to each client's needs post-discharge, ensuring a smooth transition and ongoing support.
  • Document all client interactions and treatment progress meticulously, adhering to company policies regarding confidentiality and record-keeping.
  • Participate actively in interdisciplinary team meetings to coordinate care and discuss treatment plans, alongside case management and utilization reviews.
  • Research community resources and advocate for clients’ access to necessary services such as health care, transportation, and financial assistance.

Qualifications
  • A Master's Degree in Social Work from an accredited graduate program is required.

Compensation and Benefits

The annual salary for this position ranges from USD 120,000 to 130,000, commensurate with experience and qualifications.
