As a Licensed Professional Counselor at Trinity Health Behavioral Center, your primary responsibilities will include:

• Conducting patient intakes, performing diagnostic and functional evaluations, and recommending treatment goals based on established plans.
• Providing brief, focused interventions for individuals seeking mental health support within a primary care setting.
• Collaborating with primary care providers to determine whether patients should remain in the primary care environment or be referred to specialized mental health services.
• Initiating follow-up communication to assess patient progress and determine if adjustments in treatment approaches are necessary.
• Developing relapse prevention plans for patients at risk and aiding them in achieving stable functioning.
• Monitoring and coordinating behavioral health services while linking patients with external providers when appropriate.
• Documenting patient progress and diagnostic information accurately for ongoing treatment planning.
• Contributing to the behavioral health services offered by consulting with clinic management and the healthcare team about outcomes and program evaluation.
• Encouraging patients to adopt self-management skills and providing education on their treatment options.
• Adhering to the ethical standards of conduct for clinical professionals and other duties as assigned.

In this role, you will support a diverse patient population, including individuals facing mental health challenges such as anxiety, depression, and trauma-related disorders. Your work will directly contribute to improved health outcomes by enabling patients to access vital mental health services in a community-oriented environment. By addressing psychosocial issues and collaborating closely with a primary care team, you will help integrate behavioral health into physical health care, ultimately fostering holistic care.

To be considered for this role, candidates must possess the following qualifications:

• A minimum of a Master’s Degree in Clinical Counseling or a related field.
• A current and valid LPC (Licensed Professional Counselor) license in Pennsylvania.
• Demonstrated knowledge of mental health assessment, treatment planning, and therapeutic techniques.
• Strong understanding of the impact of mental and physical health issues on individuals and communities.
• Ability to establish and maintain effective relationships with various stakeholders, including patients, families, and healthcare professionals.
• Proficiency in maintaining clinical records and preparing reports accurately and timely.
• Capacity to express opinions clearly and collaborate effectively within an interdisciplinary team.

While not mandatory, the following experiences are preferred:

• Bilingual proficiency in English and Spanish, enhancing communication with a broader patient demographic.
• Experience working within community health clinics or Federally Qualified Health Centers (FQHCs).
• Previous exposure to integrated health care settings or team-based care models.
