In this fully remote role, you will work flexibly while connecting with clients throughout the Bethlehem, Pennsylvania area. • Conduct comprehensive biopsychosocial assessments tailored to each patient, ensuring that evaluations are culturally responsive and informative for the treatment transition within the primary care setting.
• Deliver a range of behavioral health services, including counseling, individualized treatment plans, crisis intervention, and support for substance abuse issues in both individual and group settings.
• Perform detailed documentation of mental health evaluations, progress notes, and crisis assessments electronically to maintain seamless communication with the healthcare team.
• Provide linkages to local community resources and services, facilitating connections to medication assistance programs and other necessary support systems.
• Collaborate with quality improvement teams to refine data collection methodologies, ensuring that all patient outcomes are effectively tracked and addressed.

Your primary focus includes a diverse clientele ranging from children and adolescents to adults and families. Given the community health context, you will work with individuals facing various challenges, including mental health disorders, substance use issues, and chronic health conditions. You will be responsible for advocating for effective treatment plans that resonate with the client’s cultural and individual needs.

• A Master’s degree in Social Work with active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania.
• A minimum of three years of post-licensure experience in a mental or clinical setting.
• Solid grounding in various counseling techniques, crisis intervention methodologies, and the ability to exercise independent judgment in delivering therapeutic support.
• Proficiency in electronic health record (EHR) management and documentation practices, ensuring compliance with industry standards.

• At least two years of experience in group facilitation, demonstrating the ability to guide discussions and support patients in a group therapy context.
• Previous involvement in program evaluation or quality improvement initiatives within healthcare settings is advantageous.
