As a member of our remote telehealth team, you will engage with patients using advanced virtual communication platforms, ensuring that quality mental health services remain accessible, regardless of a patientโ€™s location. Key Responsibilities
  • Conduct comprehensive patient intakes and assessments, focusing on diagnostic and functional evaluations to inform treatment plans.
  • Collaborate closely with primary care providers to identify, treat, and manage mental health and psychosocial issues, actively participating as a key member of the primary care team.
  • Provide brief therapeutic interventions to patients requiring mental health support, drawing from a range of evidence-based practices and methodologies.
  • Recommend treatment modalities and make referrals to specialty mental health facilities as applicable, ensuring the best care pathways for each client.
  • Initiate and document follow-up communications with clients to assess progress, make adjustments to treatment as needed, and maintain treatment adherence.
  • Develop relapse prevention plans and supportive strategies to help clients maintain stable functioning and promote long-term wellbeing.
  • Monitor and coordinate behavioral health services delivery, ensuring seamless communication with other treatment providers, both within and external to our clinic.
  • Maintain accurate clinical documentation, including treatment charts and progress reports, to comply with agency and Federal Qualified Health Center (FQHC) standards.
  • Assist in the education and empowerment of clients regarding self-management skills necessary for participation in their own treatment journey.
  • Work collaboratively with clinic management and team members to enhance behavioral health service delivery and program evaluation.
  • Uphold the Code of Ethical Conduct for Clinical Professionals at all times.
  • Perform additional duties as assigned to support the clinicโ€™s operational needs.
Patient Population / Client Focus

This role focuses on serving a diverse and multifaceted client population, including individuals experiencing various mental health challenges, psychosocial stressors, and those in need of preventative interventions. Qualifications

Compensation and Benefits

The yearly salary for this position ranges from $87,000 to $105,000, commensurate with experience and qualifications.
