The Veteran's Communicare Therapeutic Clinic is seeking both full-time and part-time Licensed Professional Counselors (LPC) to join our dedicated team. In this role, you'll provide vital support and therapeutic services to individuals facing various mental health challenges, helping them to improve their emotional, physical, and mental well-being. You will collaborate closely with primary care providers and other healthcare professionals to design effective treatment plans and promote holistic health.

In response to current healthcare demands, this position is primarily remote, allowing for flexible scheduling based on patient appointments. Despite the remote nature of the role, counselors will be part of an integrated outpatient clinic team dedicated to providing comprehensive behavioral health services. You will work in a supportive and collaborative environment, engaging in regular consultations with primary care teams to ensure cohesive patient care.

• Collaborate closely with primary care providers to identify and address patients' mental health and psychosocial issues, reinforcing the clinic's patient-centered approach.
• Conduct thorough patient assessments focusing on diagnostic and functional evaluations, offering informed recommendations regarding treatment goals and plans.
• Implement targeted interventions for patients seeking mental health support, utilizing evidence-based practices to guide your approach.
• Guide primary care providers in determining appropriate treatment pathways for patients, including referrals to specialized mental health resources when necessary.
• Facilitate follow-up procedures to monitor patients' progress and adjust treatment strategies accordingly to enhance engagement and effectiveness.
• Develop and oversee relapse prevention plans to support patients in achieving and maintaining stability in their mental health.
• Identify at-risk patients and strategize preventive measures to mitigate the escalation of their conditions.
• Coordinate healthcare services for patients within the behavioral health spectrum, forging connections with other providers as necessary with patient consent.
• Document patient progress and clinical assessments systematically in treatment charts, maintaining an accurate record of care provided.
• Maintain open communication with team members and primary care providers, ensuring all parties are informed about patients' treatment needs and progress.
• Assist patients in making positive behavioral changes, including those with comorbid physical conditions, to enhance their overall health.
• Empower patients with self-management strategies and educational resources to actively participate in their recovery journey.
• Provide consultation to clinic management concerning behavioral health services, contributing to program evaluation and improvement initiatives.
• Assist patients in adhering to any prescribed medical treatments, including managing side effects from medications.
• Adhere diligently to the Code of Ethical Conduct for Clinical Professionals, upholding the highest standards in patient care.
• Perform any additional duties as required to support clinic operations.

You will work with a diverse patient population, primarily consisting of individuals facing mental health challenges ranging from anxiety and depression to more complex conditions. • A Master’s Degree in Clinical Counseling is required.
• Current licensure as a Licensed Professional Counselor (LPC).
• Strong understanding of mental health assessments, treatment planning, and diverse therapeutic methodologies including family and group therapy.
• Knowledge of DSM-IV diagnoses and behavioral therapy techniques.
• Ability to effectively communicate in both English and Spanish is preferred but not mandatory.
• Strong mental and physical health to fulfill the demands of the role.
• Comprehensive understanding of the social implications of mental health conditions.
• Familiarity with community resources and their organization is advantageous.

• Prior experience in a clinical or outpatient setting is beneficial.
• Experience with electronic health record systems.
• An understanding of healthcare regulations, including those pertaining to Federally Qualified Health Centers (FQHC).

The annual salary for this position ranges from USD 87,000 to 105,000, commensurate with experience and qualifications.
