Peer Support Behavioral Center is actively seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join our team in a remote capacity. This position is entirely remote, allowing you the flexibility to balance work and personal commitments while ensuring that you remain an integral part of our dedicated healthcare team. Operating through telehealth, you will engage with patients in a virtual setting, using advanced technology to deliver high-quality counseling services. You will work alongside other healthcare professionals, collaborating to provide comprehensive care while utilizing electronic health record systems to document patient progress efficiently.

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial challenges faced by patients, acting as a proactive member of the healthcare team.
• Conduct thorough patient intakes, focusing on comprehensive diagnostic and functional evaluations before making informed recommendations for treatment plans to the primary care provider.
• Provide brief, targeted interventions for patients requiring immediate mental health services, addressing their emotional and psychological needs effectively.
• Assess patient treatment suitability for continued management within the primary care context or recommend referrals to specialized mental health facilities as needed.
• Follow up with patients to monitor their progress, adjusting treatment approaches and providing additional support as necessary.
• Develop tailored relapse prevention plans, assisting patients in maintaining stability and managing their mental health proactively.
• Identify patients who may be at risk for worsening mental health conditions, implementing strategies to prevent deterioration.
• Coordinate health services related to behavioral health, ensuring efficient communication with other treatment providers inside and outside of the primary health network while respecting patient confidentiality.
• Document all interactions and treatment progress meticulously in the electronic health record system to maintain accurate patient histories.
• Keep primary care providers informed of patient needs, outcomes, and progress to facilitate collaborative treatment planning.
• Support patients in making healthier lifestyle choices and adapting behaviors to improve their overall health, especially in cases where mental health overlaps with physical health issues.
• Implement self-management skills and educational resources that empower patients to take an active role in their recovery journey.
• Consult with clinic management and team members regarding behavioral services, contributing insights for program evaluation and enhancement.
• Assist patients in adhering to medical treatments initiated by their primary care physicians, offering strategies to manage side effects and improve compliance.
• Uphold professional ethical standards as outlined in the Code of Ethical Conduct for Clinical Professionals.
• Perform additional duties as assigned by management to support clinic operations and patient care.

The ideal candidate will work with a diverse clientele, including individuals facing varying degrees of mental health challenges, such as anxiety, depression, trauma, and other psychological disorders. • A minimum of a Master’s Degree in Clinical Counseling or a related field is required.
• You must hold a current LPC (Licensed Professional Counselor) license to practice in Pennsylvania.
• Proficient knowledge of mental health assessments, treatment strategies, and diagnostic criteria (DSM-IV) is essential.
• Ability to employ various psychotherapeutic techniques and demonstrate sound clinical judgment.
• Excellent communication and interpersonal skills to effectively engage with patients and healthcare team members.
• Knowledge of community resources and services to facilitate comprehensive patient care.

• Bilingual proficiency in English and Spanish is preferred but not mandatory, enhancing communication with our diverse patient population.
• A strong understanding of the personal and social impacts of mental and physical health issues on individuals and families is advantageous.
• Previous experience working in a telehealth capacity is beneficial.

This position offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with qualifications and experience.
