About the Job:

The Manager, Digital Population Health Applications Transformation is responsible for leading the planning, delivery, and adoption of enhancements, optimization efforts, and transformational initiatives across the full compliment of Epic DPH Applications, including Epic Healthy Planet, MyChart, Cheers, Hello World, Welcome, EpicCare Link, and related digital population health capabilities. This role advances the digital front door and patient engagement strategy by enabling seamless, intuitive, and connected digital experiences that span patient access, communication, engagement, and longitudinal health management. In parallel, the Manager leads the evolution of population health applications to support proactive care management, quality performance, and value-based care outcomes. Through disciplined project delivery, workflow redesign, platform optimization, automation, and modernization, the Manager drives improvements in patient experience, care coordination, and population health outcomes. The Manager partners closely with Population Health, Clinical, Access, and operational leaders, as well as IT peers, to translate strategic priorities and business needs into executable roadmaps, funded initiatives, and measurable outcomes.

  • Provide direct leadership and accountability for assigned teams, services, or functional areas, including setting priorities and managing daily execution
  • Supervise, assign work, evaluate performance, provide feedback, and support professional development of staff
  • Manage operational issues, incidents, and service disruptions within scope, escalating as necessary to protect patient care, compliance, or business operations
  • Ensure assigned services comply with organizational policies, regulatory requirements, and governance standards

Who You Are:

Education: Bachelor's degree in Healthcare, Information Technology, Business, or related field (or equivalent substantial progressive work experience, certifications, and education), Required
Licensure/certification: ITIL Foundation Certification within 6 months of hire Must possess an active Epic certification in at least one of the following: MyChart, Healthy Planet, Cheers, HIM, Required

Experience: A minimum of five (5) years of combined IT/Healthcare experience, of which included a minimum of three (2) years of experience managing IT Infrastructure., Required
