UPLIFT HOMECARE, LLC is seeking a compassionate and dependable Licensed Physical Therapist Assistant (PTA) to join our growing home health team serving Ruskin and surrounding communities.


This position offers the opportunity to provide one-on-one patient care in the comfort of patients homes while working alongside a supportive clinical team dedicated to improving quality of life and promoting independence.

Position Summary

The Licensed Physical Therapist Assistant provides skilled therapy services under the supervision of a Registered Physical Therapist. The PTA is responsible for implementing treatment plans, monitoring patient progress, educating patients and caregivers, and maintaining accurate clinical documentation in accordance with physician orders and agency policies.


Responsibilities

  • Implement physical therapy services according to the treatment plan established by the supervising Physical Therapist and physician.
  • Provide therapeutic exercises, treatments, and interventions to relieve pain and restore function.
  • Observe, document, and report patient responses and condition changes to the supervising PT and Clinical Manager.
  • Assist in evaluating patient functional abilities and revising care plans as needed.
  • Complete clinical notes and progress reports accurately and within required timeframes.
  • Participate in care coordination conferences and interdisciplinary communication.
  • Educate patients and caregivers on exercises, mobility, safety, and treatment goals.
  • Participate in quality improvement initiatives and in-service education programs.
  • Maintain current knowledge of physical therapy practices and standards.


Qualifications

  • Current Florida Physical Therapist Assistant (PTA) license
  • Graduate of an accredited Physical Therapist Assistant program
  • Successful completion of the national PTA examination
  • Minimum of 1 year of home health experience required
  • Strong communication and documentation skills
  • Current CPR Certification
  • Valid drivers license, reliable transportation, and current auto insurance
  • Ability to travel to patient homes throughout Riverview and surrounding areas


Work Environment

This position requires frequent travel to patient homes with varying home environments and conditions. Candidates must be able to safely perform patient care duties while maintaining professionalism and adaptability in diverse settings.
