• Conduct thorough assessments to understand the unique needs of each patient and tailor therapeutic interventions accordingly.
• Perform individual and family counseling sessions to provide emotional support and coping strategies for patients facing significant life challenges.
• Collaborate with healthcare professionals and community resources to coordinate overall patient care and support transitions.
• Advocate for patients’ rights and access to necessary resources, ensuring they receive comprehensive care and assistance.
• Maintain diligent documentation utilizing EMR tools to track patient progress, treatment plans, and outcomes effectively.
• Participate in interdisciplinary team meetings to discuss patient concerns, treatment strategies, and ensure all aspects of care are addressed.

• Engage in family conferences and care transition discussions to facilitate seamless handovers and continuity of care.
• Address financial and social needs of patients, connecting them with community support resources as required.
• Maintain ongoing communication with patients to ensure their evolving needs are met and adjust treatment plans as necessary.

Our patient population includes individuals experiencing varying degrees of mental health challenges, often exacerbated by chronic or terminal illnesses. The therapist will engage with patients of all ages and their families, focusing on empowering them through evidence-based therapeutic modalities. • A Master’s or Doctorate degree in clinical psychology, counseling, or clinical social work (e.g., MA, MSW, MFT, MHC, PhD, PsyD) from an accredited program.
• An active and independent license to practice in the state of Oregon, including LCSW, LMFT, or LPC credentials.
• Demonstrated proficiency in using electronic medical records systems and effective electronic communication methods.
• Strong organizational skills and the ability to manage a substantial case load while collaborating effectively with multi-disciplinary teams.

• Experience working in a remote setting or telehealth environment.
