As an LCSW at Sunny Valley Psychotherapy Center, you will undertake various responsibilities that are crucial to the well-being of our patients, including:

• Conducting thorough assessments of clients' counseling needs and substance abuse diagnoses utilizing DSM criteria to determine the appropriate level of care based on ASAM standards.
• Developing comprehensive treatment plans tailored to meet the individual needs of clients, particularly in the realm of substance abuse counseling.
• Creating a detailed discharge and continuum of care plan to guide clients towards recovery pathways that ensure their movement to suitable levels of care.
• Documenting all counseling, educational services, assessments, and follow-ups meticulously within the client’s clinical record, ensuring compliance with best practices.
• Managing a caseload of clients, including ongoing case management and regular updates for utilization review, fostering an organized care approach.
• Coordinating with family members and support services to ensure a holistic support system for clients, carefully recording all interactions and services within clinical notes.
• Facilitating interventions for crisis-related issues using a multidisciplinary team approach, ensuring timely and effective response to client needs.

Our center specializes in serving clients dealing with both substance use disorders and mental health challenges, particularly within the hospice environment. You will work with diverse populations, providing compassionate and effective counseling to individuals and families facing the complexities of addiction alongside terminal illnesses. The focus will be on fostering long-term recovery and enhancing the quality of life during challenging times.

The successful candidate for this LCSW position will have:

• A Bachelor’s Degree in Social Work, with a preference for candidates holding an LCSW licensure.
• Demonstrated professionalism, strong therapeutic boundaries, and a commitment to cultural competency in working with a wide range of clients.
• Excellent organizational and time-management skills to manage diverse client loads effectively.
• A self-motivated and compassionate approach to helping clients navigate their recovery journeys.

Ideal candidates will have experience specifically working in a hospice or palliative care setting. Familiarity with co-occurring disorders and the ability to utilize different therapeutic modalities to cater to client needs will be advantageous. Experience in documentation and compliance with clinical standards is also preferred.
