Key Responsibilities
  • Conduct detailed and ongoing assessments for patients to identify their unique needs and provide tailored therapeutic interventions.
  • Actively communicate with supervisors, direct care staff, family members, and other stakeholders regarding each patient’s progress and care plan.
  • Prepare and provide a tentative schedule to supervisors encompassing patient appointments, home visits, and observations to facilitate monitoring efforts.
  • Coordinate and execute home visits as a part of in-home observations; perform thorough chart reviews and engage with staff during interviews for comprehensive data analysis.
  • Ensure the effective implementation of the program plan, verifying all necessary actions are taken in alignment with treatment goals.
  • Participate actively in Interdisciplinary Team Meetings and contribute valuable insights during relevant discussions.
  • Document monthly and quarterly progress notes detailing patient developments and milestones.
  • Organize and conduct training for staff to ensure all team members are proficient in implementing therapy plans and associated documentation processes.
  • Facilitate follow-up interviews, home visits, and support observations as necessary to continually address the evolving needs of each patient.
  • Maintain comprehensive records to ensure information is easily accessible for inquiry by authorized personnel.
  • Serve as a liaison with the broader public mental health community, aiding in the development of goals and resources.
  • Assist in the creation and refinement of training materials aimed at enhancing programmatic interventions.

You will work with a diverse range of clients, including both adults and youth, presenting various mental health challenges. Qualifications
