This position operates in a remote setting, providing flexibility while maintaining a strong network of support among team members. Your work will directly involve coordinating care with our Hospice Center, where we strive to address the needs of patients facing life-limiting illnesses. Collaboration with other healthcare professionals, including nurses and physicians, will be essential in fostering an integrated approach to patient care.

• Conduct comprehensive assessments and develop individualized treatment plans based on each client's unique needs.
• Implement therapeutic interventions, facilitating both individual and group therapy sessions to encourage emotional expression and healing.
• Utilize evidence-based practices, continually adjusting approaches based on patient progress and feedback.
• Maintain active communication with interdisciplinary teams to monitor and evaluate client outcomes effectively.
• Document all patient interactions and maintain accurate, timely records in electronic health record (EHR) systems.
• Provide ongoing support to families, enhancing their understanding and skills in coping strategies for their loved ones.
• Participate in case reviews and contribute to team meetings to share insights and best practices.

Your primary focus will be working with a diverse client population, particularly those within the hospice care framework. This role involves addressing not only the mental health needs of patients but also their emotional and spiritual well-being. You will engage with individuals experiencing grief, existential concerns, and adjustments related to chronic illness as well as providing critical support to their families during challenging times.

• A Master’s degree in Social Work or a closely related field is required.
• An active Licensed Clinical Social Worker (LCSW) license in the state of California.
• A minimum of two years of clinical practice experience, particularly in mental health or hospice settings.
• Proficient in utilizing electronic health record (EHR) systems, with advanced computer skills for documentation.

• Experience working in end-of-life care or within a hospice setting is highly desirable.
• Relationships with local healthcare providers and community resources that can enhance care options for clients.
• Strong background in group therapy and systemic family therapy methodologies.
