This position is remote, allowing you the flexibility to manage your workload effectively while maintaining a work-life balance. You will operate from your home office but stay connected to the team and clients through secure, virtual platforms. Our outpatient office environment provides you with the tools and support necessary to deliver the highest standard of care. • Conduct thorough intake assessments and comprehensive psychosocial evaluations to understand the emotional, mental, and social factors influencing each client's well-being.
• Develop personalized care plans in collaboration with clients and their families, ensuring these plans reflect individual preferences, cultural backgrounds, and personal goals.
• Provide emotional support and counseling services via telehealth sessions to assist seniors coping with anxiety, depression, or transitions associated with aging.
• Advocate for clients’ access to vital community resources, ensuring they are aware of and able to utilize programs that can support their mental and physical health.
• Collaborate with an interdisciplinary team to coordinate care, ensuring seniors receive comprehensive services that address all aspects of their health.
• Monitor clients' ongoing well-being through regular follow-ups and virtual check-ins, assessing living conditions and making recommendations for any necessary changes.
• Facilitate family meetings to resolve conflicts and enhance communication among family members involved in the senior’s care.
• Educate clients and their families on available community programs and support groups, enhancing their engagement with social and mental health resources.
• Maintain meticulous records in electronic health record (EHR) systems, documenting client assessments, treatment plans, and progress notes to track outcomes and ensure compliance with ethical standards.
• Continuously update your knowledge on social work interventions, policies, and best practices to enhance service delivery and client care.

This role primarily focuses on senior clients, often experiencing complex psychosocial challenges related to aging. Whether they face issues such as loneliness, health concerns, or cognitive decline, your expertise will guide their families in finding the right support systems. Understanding that seniors have unique needs, you will address these with compassion, utilizing targeted interventions to uplift their quality of life.

• A Master's degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW-C) in the state of Maryland.
• A minimum of two years of experience in social work, preferably in a geriatric or community-centered setting.
• Strong understanding of social work principles, aging issues, and the availability of community resources.
• Demonstrated skills in assessment, counseling, and strategic problem-solving.
• Ability to empathize and establish rapport with elderly clients and their families, fostering trust and support.
• Excellent communication and interpersonal skills to collaborate effectively with interdisciplinary teams.
• Familiarity with electronic health record systems and proficiency in computer applications essential for remote service delivery.

• Experience working in outpatient mental health or community health settings with a focus on the senior population.
• Knowledge of local community resources available for seniors, such as mental health programs and assistance services.
• Experience conducting assessments and developing care plans tailored to the needs of older adults.
