• Conduct thorough clinical assessments and develop personalized treatment plans tailored to each patient’s unique needs and circumstances.
• Utilize evidence-based therapeutic interventions, including solution-focused therapy and behavioral approaches, to foster effective coping mechanisms among patients.
• Maintain meticulous and timely documentation in compliance with both organizational standards and regulatory requirements to ensure the continuity of care.
• Collaborate closely with physicians, nurses, and other healthcare professionals within the hospice team to discuss treatment strategies and patient progress.
• Provide education, resources, and emotional support to patients and their families, helping them navigate the complexities of end-of-life care.
• Regularly engage in professional development by staying informed about the latest research and practices in mental health to enhance treatment approaches.

In this role, you will primarily serve patients who are experiencing chronic illness and require emotional and psychological support as they navigate their hospice care journey. You will work with a diverse population that includes patients of various ages and backgrounds, providing compassionate care that acknowledges their individual experiences and needs during this sensitive phase of life.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of New York, enabling you to provide clinical supervision.
• Demonstrated expertise in solution-based psychotherapy interventions and an understanding of hospice care dynamics.
• Exceptional communication, empathy, and problem-solving skills, with an ability to connect with patients and their families.

• Previous experience in a hospice, palliative care setting, or a similar healthcare environment is highly desirable.
• Familiarity with telehealth platforms and remote counseling methodologies.
• Training in trauma-informed care or grief counseling is advantageous, as you will often be addressing sensitive subjects with patients and loved ones.

• The position offers a competitive salary range of $90,000 to $100,000 per year, commensurate with experience and qualifications.
