Triage RN Coach (Part-time)

Location: Fully Remote

Works Schedule: 2:45pm - 11:15pm | 4 Shifts a Week | Rotating Weekends

At Novant Health, our Triage RN Coaches provide essential after-hours clinical support for more than 600 practices—both Novant Health and independent offices. As part of our Nurse Triage Line and Care Connections Team, you'll be the clinical expert patients turn to when they're experiencing symptoms at home, in nursing facilities, or wherever they may be.

In this fully remote role, you'll assess symptoms, determine the safest and most appropriate plan of care, and guide patients through next steps—whether that means calling 911 immediately, scheduling follow-up care, or managing symptoms safely at home. What You'll Do
  • Provide Telephonic Nurse Triage: Assess symptoms using approved clinical protocols to determine the safest plan of care—from immediate emergency intervention to home-care guidance.
  • Serve as After-Hours Clinical Support: Act as the clinical resource for 600+ offices, responding to patient needs when primary clinics are closed.
  • Collaborate with CCS I Team: Receive warm transfers from Care Connection Specialists who collect patient demographics and initial complaints.
  • Guide Patients Through Care Decisions: Help callers understand their options and navigate the healthcare system appropriately.
  • Support Acute & Chronic Care Needs: Deliver evidence-based guidance for a wide range of conditions, ensuring high-quality and safe care recommendations.
  • Communicate with Providers: Reach out to physicians, advanced practice providers, or on-call staff as needed to coordinate next steps.
  • Manage Clinical Information: Receive and relay radiology and lab results, provide patient counseling, and answer general health questions within scope.
  • Promote Patient Engagement: Encourage appropriate venue of care and help reduce unnecessary ED visits or hospitalizations.
  • Document Thoroughly & Accurately: Record assessments, recommendations, and communications in the clinical system of record.
  • Maintain Scope: Provide clinical recommendations and health education without diagnosing or stating what condition a patient "has."
What We're Looking For
  • Education: 4 Year / Bachelors Degree, preferred.
  • Experience: 5 years of Clinical experience in hospital, home health, ambulatory or community setting with a recent focus on acute and/or chronic-care management, required.
  • Licensure/Certification: Current unrestricted RN license for the states in the NH footprint, required.
