Are you an LPN/LVN who enjoys using your clinical knowledge to help patients better understand and manage their medications-but prefers a structured, telephonic environment over traditional bedside care?

We are hiring LPN Clinical Interviewers to conduct medication-focused outreach with health plan members. In this role, you will speak with members to understand how they are taking their medications, identify barriers to adherence and safe medication use, provide education within your scope of practice, and escalate concerns to the appropriate clinical teams.

This is a fully remote, high-volume clinical role focused primarily on outbound calls and documentation. You will not maintain a consistent patient caseload or develop long-term relationships with the same members. Instead, you will make an impact across a broader population by helping members improve medication adherence, safety, and health outcomes.


What You'll Do
  • Conduct telephonic medication interviews to support Medication Therapy Management (MTM) activities.

  • Verify member identity and obtain appropriate consent before discussing medication information.

  • Review and reconcile prescription, OTC, herbal, and supplement medications.

  • Ask questions regarding medication adherence, effectiveness, side effects, affordability, and barriers to proper use.

  • Identify medication-related concerns and accurately document findings.

  • Provide clear, easy-to-understand medication education within your scope of practice.

  • Reinforce appropriate medication administration, dosing, storage, and adherence strategies.

  • Recognize urgent, complex, or out-of-scope concerns and escalate appropriately.

  • Partner with pharmacists, care coordinators, and operational teams to support follow-up.

  • Document member interactions, assessments, and outcomes in designated systems.

  • Follow established clinical workflows, scripts, decision trees, and escalation pathways.

  • Maintain documentation that meets clinical, regulatory, and audit standards.

  • Navigate multiple systems simultaneously, including MTM platforms, call systems, email, and documentation tools.

  • Manage outreach, documentation, and follow-up activities efficiently.

  • Participate in quality reviews, coaching, audits, and performance activities.


Types of Medication Reviews

Comprehensive Medication Reviews (CMR)
Conduct in-depth reviews of a member's prescription medications, OTC medications, vitamins, and herbal supplements to identify potential medication-related concerns, adherence barriers, side effects, and opportunities for improved medication management.

Targeted Medication Reviews (TMR)
Conduct focused outreach based on identified medication safety risks or therapy gaps, such as missed refills, non-adherence, duplicate therapies, or potentially inappropriate medications.


What We're Looking For
Required
  • Graduate of an accredited LPN/LVN program.

  • Active LPN/LVN license in good standing.

  • Healthcare and/or telephonic outreach experience.

  • Comfortable working in a high-volume, productivity-focused environment.

  • Strong ability to navigate electronic documentation systems and multiple software platforms.

  • Proficiency with Microsoft Office tools.

  • Spanish language proficiency required.

  • Strong communication, organization, documentation, and patient engagement skills.


Preferred
  • Medication Therapy Management (MTM) experience.

  • Transition of Care experience.

  • Experience working with Medicare or Medicaid populations.

  • Bilingual Spanish/Vietnamese candidates strongly preferred.


Important: Is This Role Right for You?

This position is not a bedside nursing role and is not traditional case management.

You may speak with many different members throughout the day and may never speak with the same member twice. The position is ideal for an LPN/LVN who enjoys:

Schedule & Training
  • Monday-Friday.

  • Hours of operation are between 9:00 AM-8:00 PM EST.

  • Multiple start times are available within the operating hours.

  • Approximately 2-3 weeks of training, including classroom instruction, call shadowing, and reverse shadowing.

  • New hires receive approximately 90 days to become comfortable with the role before performance is assessed relative to the broader team.


Remote Work Requirements
  • Fully remote position.

  • Client provides computer, monitor, peripherals, and webcam.

  • Candidates must have a HIPAA-compliant workspace, including a private/closed-door area and appropriate headphones.

  • Reliable internet connection is required.
