About the Job:

  1. Patient Care Coordinator - Health Coach.
  2. We are seeking a Patient Care Coordinator - LVN/LPN or RN .
  3. Must have a current nursing license.
  4. You will coordinate a care and coaching program with remotely monitored patients.
  5. This program focuses on diet and exercise. An increased knowledge based of macronutrients,
  6. calories, strength training, cardio, etc. is preferred.
  7. This is a 100% work from home position. Anywhere in the USA.
