Key Responsibilities
  • Conduct thorough patient intakes, focusing on diagnostic assessments and functional evaluations, to develop comprehensive treatment plans together with primary care providers.
  • Provide focused, short-term interventions for patients requiring immediate mental health support, ensuring adherence to established Agency and Federally Qualified Health Center (FQHC) guidelines.
  • Monitor and document each patientโ€™s clinical progress in treatment charts, ensuring accurate records and updates that contribute to their ongoing care.
  • Collaborate closely with primary care providers to identify patients needing specialty care and assist in the referral process as necessary.
  • Engage with patients for follow-up appointments to monitor their well-being and make adjustments to treatment plans as required.
  • Work with patients to create and implement relapse prevention strategies that promote long-term stability and recovery.
  • Identify 'at-risk' patients and assist in developing proactive plans to avert potential setbacks in their mental health journey.
  • Advise clinic management and work collaboratively with other team members to enhance behavioral services and improve program outcomes.
  • Educate patients about self-management techniques and empower them to take an active role in their treatment and recovery.
