This position is entirely remote, offering counselors the flexibility to manage their schedules while delivering high-quality care through telehealth services. Utilizing modern technology and platforms tailored for virtual consultations, LPCs will engage with clients in a supportive and comfortable atmosphere. Our telehealth system ensures seamless communication and documentation while prioritizing patient confidentiality and security.

• Conduct thorough patient intakes, focusing on diagnostic assessments and functional evaluations, to develop comprehensive treatment plans together with primary care providers.
• Provide focused, short-term interventions for patients requiring immediate mental health support, ensuring adherence to established Agency and Federally Qualified Health Center (FQHC) guidelines.
• Monitor and document each patient’s clinical progress in treatment charts, ensuring accurate records and updates that contribute to their ongoing care.
• Collaborate closely with primary care providers to identify patients needing specialty care and assist in the referral process as necessary.
• Engage with patients for follow-up appointments to monitor their well-being and make adjustments to treatment plans as required.
• Work with patients to create and implement relapse prevention strategies that promote long-term stability and recovery.
• Identify 'at-risk' patients and assist in developing proactive plans to avert potential setbacks in their mental health journey.
• Advise clinic management and work collaboratively with other team members to enhance behavioral services and improve program outcomes.
• Educate patients about self-management techniques and empower them to take an active role in their treatment and recovery.

The counseling services provided at Mohawk Trail Counseling Center target a diverse clientele. LPCs will support individuals facing various mental health challenges, from mild anxiety and depression to more complex psychosocial issues. By fostering an empathetic and trusting environment, counselors are able to facilitate meaningful connections that help patients navigate their mental health journeys.

• Master's Degree in Clinical Counseling or a related field from an accredited institution.
• Current and valid LPC (Licensed Professional Counselor) license.
• In-depth knowledge of mental health assessment techniques, treatment planning, and evidence-based therapeutic modalities.
• Understanding of DSM-IV diagnoses and familiarity with psychotherapeutic techniques.
• Strong ability to document clinical progress meticulously and maintain comprehensive records.

• Bilingual proficiency in English and Spanish is preferred but not mandatory.
• Experience working in telehealth or remote counseling environments is advantageous.
• Knowledge of community resources that can further support clients’ mental health needs.
• Prior experience collaborating within an interdisciplinary team setting.
