- Lead and facilitate therapeutic group sessions and workshops focused on relevant topics such as coping strategies, end-of-life issues, and personal empowerment.
- Collaborate with diverse hospital departments to ensure continuity of care, fostering relationships with team members in patient support services and community organizations.
- Engage in proactive outreach initiatives to raise mental health awareness and promote wellness events within the hospice community.

Qualifications- Active New York state license as either a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).
- A Bachelor's degree in social work, psychology, counseling, or a related field is required; higher degrees are preferred.
- In-depth knowledge and understanding of the mental health challenges associated with terminal illness and end-of-life care.
- Exceptional communication and interpersonal skills, with a capacity to engage empathetically with patients from diverse backgrounds.
- Proven familiarity with evidence-based practices and brief therapy models, particularly suited to a hospice care setting.
- Ability to effectively manage a high-volume, varied caseload, demonstrating exceptional organizational skills and flexibility.
- At least 2 years of clinical experience, particularly in hospice, palliative care, or a hospital-based setting.
- Experience in leading therapeutic groups and workshops.
- Familiarity with community organization collaboration and outreach initiatives.
