• Conduct comprehensive assessments and screenings to evaluate each patient's mental health needs. Develop tailored treatment plans that address both psychological and emotional aspects.
• Deliver short-term individual counseling and crisis interventions, helping patients navigate various emotional challenges such as grief, anxiety, and depression.
• Lead and facilitate therapeutic group sessions and workshops focused on relevant topics such as coping strategies, end-of-life issues, and personal empowerment.
• Collaborate with diverse hospital departments to ensure continuity of care, fostering relationships with team members in patient support services and community organizations.
• Engage in proactive outreach initiatives to raise mental health awareness and promote wellness events within the hospice community.
• Maintain meticulous and confidential patient records in adherence to ethical guidelines and legal standards, ensuring the integrity of therapy outcomes and patient information.

This role primarily centers on serving patients within a hospice setting, which presents unique challenges and dynamics. You will work with individuals facing end-of-life issues, providing them with the necessary emotional support to cope with their circumstances. Understanding terminal illness, its psychological impact, and providing compassionate care will be at the forefront of your practice.

• Active New York state license as either a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).
• A Bachelor's degree in social work, psychology, counseling, or a related field is required; higher degrees are preferred.
• In-depth knowledge and understanding of the mental health challenges associated with terminal illness and end-of-life care.
• Exceptional communication and interpersonal skills, with a capacity to engage empathetically with patients from diverse backgrounds.
• Proven familiarity with evidence-based practices and brief therapy models, particularly suited to a hospice care setting.
• Ability to effectively manage a high-volume, varied caseload, demonstrating exceptional organizational skills and flexibility.

• At least 2 years of clinical experience, particularly in hospice, palliative care, or a hospital-based setting.
• Experience in leading therapeutic groups and workshops.
• Familiarity with community organization collaboration and outreach initiatives.
