As a fully remote position based in Lakewood, New Jersey, this role provides you with the flexibility to work from home while delivering essential care and services to our patients. You will engage with a multidisciplinary team that collaborates closely to provide top-quality care within the hospice setting. • Conduct comprehensive evaluations and assessments to determine the needs of patients and their families.
• Develop, implement, and monitor Individualized Family Service Plans (IFSP) that cater to the specific needs of each family.
• Provide emotional and practical support to families navigating the challenges of hospice care.
• Collaborate with healthcare professionals to ensure interdisciplinary approaches in the care process.
• Administer evidence-based evaluations and interventions tailored to individual patients.
• Foster open communication and develop rapport with patients, families, and colleagues to enhance a supportive environment.
• Deliver coaching and support to families using natural learning environment techniques to improve children’s developmental outcomes.
• Ensure compliance with all Federal and State regulations governing hospice care.
• Document patient progress and family interactions meticulously to contribute to continuous care improvement.

Your primary focus will be on children with disabilities or developmental delays along with their families. You will provide early intervention services that support the child's development in critical areas such as physical, cognitive, social/emotional, and communication skills. Your work will directly impact young lives by offering essential resources and strategies to families in challenging situations, enhancing both the child's and family's quality of life.

• A Master’s Degree or Doctorate in Social Work from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.
• A valid driver’s license is preferred for any necessary community visits.
• Demonstrated ability to adapt to diverse learning styles and foster inclusivity.
• One year of relevant experience in early intervention or hospice settings is essential.
• Commitment to implementing Natural Learning Environment Practices in patient care.
• Strong organizational, time management, and communication skills are essential.
• Bilingual candidates are encouraged to apply.

• Previous work in hospice or palliative care settings will be advantageous.
