• Conduct thorough psychosocial assessments for patients facing terminal illnesses, considering emotional, social, and financial aspects.
• Develop and implement individualized treatment plans to address patients’ emotional and psychological needs based on thorough assessments.
• Utilize evidence-based therapeutic modalities to facilitate sessions aimed at coping strategies, grief counseling, and family support.
• Maintain precise and confidential documentation of patient interactions, treatment interventions, and progress notes in compliance with HIPAA guidelines.
• Collaborate regularly with interdisciplinary treatment teams to discuss patient care strategies, ensuring holistic approaches to care.
• Stay abreast of current research, best practices, and regulatory standards in social work and hospice care, adapting approaches as needed.

You will engage primarily with patients who are experiencing end-of-life care, along with their families and caregivers. This population often faces complex ethical and emotional challenges, including grief, anxiety, and existential concerns. Your role will be crucial in providing compassionate support and creating a safe, trusting atmosphere for patients and families, ultimately facilitating more peaceful transitions during difficult times.

• A Master’s degree in Social Work from an accredited program.
• Current and active LICSW (Licensed Independent Clinical Social Worker) license in Massachusetts.
• Strong interpersonal communication abilities, with a focus on empathy and listening skills.
• Proven experience in a clinical setting, particularly with hospice, palliative care, or mental health services.
• Ability to work well in a remote team environment and manage your schedule efficiently.

• Familiarity with electronic health record (EHR) systems and documentation practices relevant to social work and hospice care.
• Experience with trauma-informed care practices and understanding of various therapeutic modalities.
• Proficiency in crisis intervention techniques and the ability to work effectively under pressure.
