Key Responsibilities

• Conduct thorough assessments of clients' social, emotional, and environmental circumstances to identify barriers to well-being.
• Develop, implement, and manage personalized service plans that outline specific goals, interventions, and methods for monitoring progress.
• Provide direct counseling and psychological support to individuals and families navigating complex life challenges, enabling them to cope and thrive.
• Engage collaboratively with clients to create actionable service plans, ensuring connectivity to appropriate community resources.
• Offer crisis intervention strategies, risk assessments, and safety planning to address urgent client needs effectively.
• Work with community agencies to coordinate services, ensuring comprehensive support for clients and advocating on their behalf.
• Participate in community outreach efforts, promoting agency services and raising awareness of social support resources available to the community.
• Collaborate within multidisciplinary teams, attending case conferences and staff meetings to foster a cohesive work environment and shared learning.

Patient Population / Client Focus

The clients served at Palm Health Medical Center primarily include individuals facing substance abuse challenges, as well as those impacted by homelessness, domestic violence, mental health issues, and other social adversities. Your role will be pivotal in ensuring that these individuals receive the support and resources they need to stabilize their situations and pursue pathways to recovery and empowerment.

Qualifications

• Master's degree in Social Work from an accredited university.
• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW).
• Demonstrated knowledge of trauma-informed care principles, social justice, and community resource navigation.
• Strong communication, cultural competence, and analytical problem-solving skills are essential for success in this role.

Preferred Experience

• Previous experience working in a social service environment or a related field is preferred.
• Familiarity with evidence-based practices in social work and mental health is a plus.

Compensation and Benefits
