Position Overview

Palm Health Medical Center is actively seeking a dedicated and compassionate Licensed Independent Clinical Social Worker (LICSW) to join our team in Springfield, Massachusetts. Key Responsibilities

• Conduct thorough assessments of clients' needs, identifying the social, emotional, and environmental factors that impact their well-being.
• Create customized service plans that outline specific goals, interventions, and methods for monitoring client progress.
• Provide individualized counseling to clients and their families, helping them navigate various life challenges, including mental health issues, housing insecurity, and domestic challenges.
• Support clients in developing personalized strategies to connect with community resources, ensuring they have access to necessary support systems.
• Utilize crisis intervention techniques, facilitate risk assessment activities, and develop safety plans when required to support at-risk clients.
• Partner with community agencies to ensure coordinated care and advocacy for clients, enhancing the overall efficacy of services provided.
• Engage actively in community outreach initiatives to raise awareness about available services and programs, helping to bridge gaps for those in need.
• Participate in regular case conferences and team meetings, contributing insights and collaborating with colleagues to enhance service delivery.

Patient Population / Client Focus

The client population served by Palm Health Medical Center includes individuals and families facing significant challenges such as homelessness, domestic violence, substance abuse issues, and poverty. Our focus is on understanding the complexities of each client’s situation and leveraging community resources to foster resilience and empowerment.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) is mandatory.
• Strong foundational knowledge in social and economic justice issues relevant to our client base.
• Commitment to employing a trauma-informed care approach in all aspects of practice.
• Exceptional verbal and written communication skills, with the ability to maintain cultural competence in diverse settings.

Preferred Experience

• Previous experience working within a social service agency or similar environment is highly desirable.
• Demonstrated experience in crisis management and conflict resolution is an asset.
• Familiarity with evidence-based practices in social work and mental health support is preferred.

Compensation and Benefits
