This role operates within a telehealth framework, allowing counselors to engage with clients remotely. • Collaborate with primary care practitioners to identify and manage mental health and psychosocial issues, thereby enhancing the overall efficiency of the patient care team.
• Conduct thorough patient intakes focused on diagnostic assessments and functional evaluations, which inform tailored therapy objectives and treatment recommendations.
• Respond to urgent mental health needs and provide critical interventions in a timely manner, ensuring clients receive the support they need during crises.
• Advise on referrals for specialized mental health services while identifying patients who may benefit from specialized care.
• Regularly conduct follow-up assessments to track patient progress, evaluate treatment effectiveness, and adjust interventions as necessary.
• Develop and implement relapse prevention strategies in collaboration with clients, ensuring they possess the tools necessary to maintain mental well-being.
• Identify patients at risk of worsened health outcomes and collaborate on preventive program development tailored to address their unique needs.
• Document patient progress and treatment plans accurately in our health record systems, ensuring compliance with agency regulations.
• Maintain ongoing communication with healthcare providers to keep them informed of patients’ needs and progress, enhancing collaborative treatment planning.
• Offer self-management skills and educational resources to empower clients in their treatment and recovery journey.
• Engage with clinic management to assess behavioral services, fostering program development and improving treatment outcomes.
• Support adherence to prescribed medical treatments initiated by primary care providers, including strategies for managing medication side effects.
• Maintain professional ethical standards as outlined by the Clinical Professionals' Code of Ethics and carry out additional assigned duties.

The role primarily focuses on clients experiencing mental health challenges that affect their ability to engage in daily life. This includes individuals with anxiety, depression, family dynamics, and more. By providing patient-centered care, counselors play a vital role in promoting mental wellness within Galveston and surrounding areas.

• A Master’s Degree in Clinical Counseling is mandatory.
• A current LPC (Licensed Professional Counselor) license is required to practice.
• Deep understanding of mental health evaluation techniques, treatment planning, and diverse therapeutic methodologies.
• Knowledge of DSM-IV diagnostic criteria, and effective group and family therapy approaches.
• Fluency in both English and Spanish is highly encouraged, enhancing our service to a diverse client base.
• Exemplary mental and physical health to manage the responsibilities of the role.
• Awareness of community resources and understanding of their implications for patients’ treatment pathways.
• Familiarity with psychodiagnostic procedures and therapeutic interventions is essential.

Experience in teletherapy or remote counseling setups is advantageous but not mandatory. Additionally, prior work in multidisciplinary healthcare teams or community health settings will be viewed favorably.
