The position is entirely remote, allowing counselors to work from their preferred locations while engaging with clients through our telehealth platforms. • Collaborate with primary care providers in recognizing, treating, and managing mental health and psychosocial issues as an integral member of the primary care team.
• Conduct comprehensive patient intakes, focusing on diagnostic and functional evaluations to inform treatment plans and goals.
• Deliver brief, focused interventions for clients requiring immediate mental health services, often addressing issues such as anxiety, depression, and trauma.
• Provide informed recommendations to primary care providers regarding patient management, identifying those who may benefit from referrals to specialized mental health facilities.
• Follow up with clients post-treatment to assess their progress and adjust treatment approaches as necessary.
• Develop and implement relapse prevention plans for patients, helping them maintain stability and wellness over time.
• Detect “at-risk” patients and devise proactive strategies to prevent escalation of their mental health conditions.
• Monitor and coordinate health service delivery related to behavioral care, facilitating communication among other treatment providers with client consent.
• Thoroughly document patient progress and diagnostic insights within treatment charts, ensuring adherence to best practices in clinical record-keeping.
• Engage in continuous communication with primary care providers to ensure aligned treatment plans are developed and maintained.
• Empower patients by teaching them self-management skills and equipping them with educational resources to enhance their active participation in recovery.
• Provide insights and recommendations to clinic management regarding program evaluation and the enhancement of behavioral services.
• Support patients in following through with any prescribed medical treatments, offering strategies to manage potential side effects.
• Uphold the utmost ethical standards as outlined in the Code of Ethical Conduct for Clinical Professionals.
• Accept additional duties as assigned, contributing to the overall functioning of the clinic.

As a Licensed Professional Counselor at Thrive Hard Behavioral Counseling Center, you will work with a diverse clientele representing various age groups and social backgrounds. • Master’s Degree in Clinical Counseling or a closely related field is required.
• Possession of a current and valid LPC (Licensed Professional Counselor) license.
• Comprehensive knowledge of mental health assessment, treatment planning, and an understanding of developmental psychopathology.
• Familiarity with family therapy, group therapy, child psychotherapy, DSM-IV diagnoses, and evidence-based behavioral therapies.
• Strong communication and interpersonal skills, with bilingual English-Spanish proficiency preferred but not mandatory.
• Sound mental and physical health suitable for managing the demands of clinical responsibilities.

• Proven experience working within a community health setting or Federally Qualified Health Center (FQHC).
• Demonstrable ability to navigate the complexities of community resources and their effective utilization.
• Experience in psycho-diagnostics and psychotherapeutic modalities will be advantageous.

The salary for this position ranges from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
