• Collaborate with primary care providers to identify, assess, and manage mental health and psychosocial issues within the clinic, ensuring a holistic approach to patient care.
• Conduct patient intakes and comprehensive diagnostic evaluations, synthesizing information to create personalized treatment plans and goals.
• Provide brief, focused interventions for patients who require immediate mental health support, incorporating evidence-based practices to enhance outcomes.
• Guide patients in understanding their treatment options and recommend appropriate referrals to specialty care when needed.
• Initiate follow-up care to monitor patient progress and adjust treatment plans based on ongoing evaluations.
• Develop relapse prevention strategies and educational resources to empower patients in maintaining stability and independence.
• Facilitate coordination between various health services and providers, ensuring comprehensive care while respecting patient confidentiality.
• Document clinical notes and patient progress accurately in treatment charts, maintaining compliance with organizational and regulatory guidelines.
• Engage in consultation with clinic management and interdisciplinary teams to enhance behavioral health services and program efficacy.
• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals while performing all assigned duties.

The LPC will serve a diverse patient population with varying mental health needs, ranging from individuals experiencing acute crises to those seeking ongoing therapeutic support. • A Master’s degree in Clinical Counseling or a related field is mandatory.
• Current LPC (Licensed Professional Counselor) license in the state of New Jersey.

• Strong understanding of DSM-IV diagnoses and different psychotherapeutic techniques.
• Proficiency in the assessment and management of patients with comorbid conditions is crucial.
• Ability to work cooperatively in an interdisciplinary team environment while maintaining effective relationships with patients, families, and community stakeholders.
• Excellent written and verbal communication skills with the ability to document clinical interactions accurately.

• Experience working within a Federally Qualified Health Center (FQHC) or similar community health setting is highly desirable.
• Bi-lingual proficiency, particularly in English and Spanish, is preferred but not required.
• Familiarity with community resources and programs that support behavioral health is a significant advantage.
