• Conduct thorough psychosocial assessments to identify the mental health needs, strengths, and resources of hospice patients and their families.
• Provide targeted individual, family, and group therapy sessions using evidence-based modalities such as cognitive-behavioral therapy and solution-focused therapy.
• Develop and implement personalized treatment plans that outline specific goals and interventions, ensuring that each plan aligns with the unique circumstances of the patient.
• Collaborate with an interdisciplinary team of healthcare professionals, including physicians, nurses, and psychologists, to coordinate holistic care for patients.
• Offer crisis intervention and risk assessment to patients experiencing acute distress, including creating safety plans as necessary.
• Maintain timely and accurate documentation in compliance with ethical and legal standards, ensuring all client interactions are recorded comprehensively.
• Facilitate engaging group therapy sessions, psychoeducation workshops, and support groups aimed at empowering patients and families dealing with the complexities of end-of-life care.

Your primary focus will be on hospice patients and their families, many of whom experience an array of emotional and psychological challenges during this critical time. This role requires a deep understanding of the mental health impacts of terminal illness, grief, and loss, as well as the ability to provide compassionate care tailored to diverse cultural backgrounds and experiences. You will play a pivotal role in supporting families in navigating through the emotional complexities of end-of-life decisions and bereavement.

• A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant experience in a mental health or outpatient setting, ideally with a focus on hospice or palliative care.
• Strong knowledge of evidence-based therapeutic approaches and a passion for providing high-quality care.
• Exceptional interpersonal, communication, and assessment skills, enabling effective engagement with clients and their families.

• Experience working with hospice or palliative care settings is highly desirable.
• Familiarity with telehealth technologies and ability to effectively engage clients remotely.
