Home Health Admissions Registered Nurse (RN)

Chicopee Service Area/ Full Time

We are seeking a high-performing Skilled Home Health RN who is confident in the field, strong in clinical judgment, and understands how to build an accurate, defensible foundation of care in the home setting.

This is not a training role. This is a role for nurses who have already mastered the rhythm, complexity, and responsibility of home health admissions.

What You Will Do

As an Admission Nurse, you will:

  • Complete comprehensive Skilled Home Health admissions in the home setting

  • Perform advanced clinical assessments across physical, psychosocial, and environmental domains

  • Identify clinical risks, safety concerns, and gaps in care at the point of entry

  • Ensure all admission documentation is complete, compliant, and clinically defensible

  • Communicate findings and significant changes promptly to physicians and care teams (within 24 hours)

  • Make sound clinical decisions that shape downstream care quality and outcomes

  • Maintain accuracy and compliance aligned with internal standards and regulatory expectations

What We’re Looking For

This role is designed for experienced SHH clinicians who have:

  • Active, unrestricted RN license

  • Minimum 1+ year of Skilled Home Health experience (required)

  • Strong, independent field nursing practice in admissions or high-acuity home visits

  • Working knowledge of HomeCare HomeBase (HCHB) preferred

  • Solid understanding of home health compliance, documentation expectations, and visit structure

  • Confidence operating independently in the field and managing full admission workflows

  • Strong assessment skills with the ability to quickly synthesize clinical information

Requirements
  • Associate or Bachelor’s Degree in Nursing preferred

  • Valid driver’s license, insurance, and reliable transportation required

  • Ability and willingness to travel up to 50% within the coverage area

  • Ability to sit, stand, bend, lift, and move intermittently; able to lift 50–100 lbs
