This position is uniquely tailored for a remote work setting, granting you flexibility in your schedule while allowing you to serve our clientele effectively. The remote hospice center environment emphasizes compassionate care, as you'll often be engaging with clients during some of their most challenging times. Your interactions with clients will be supported by state-of-the-art telehealth technologies, ensuring seamless communication and therapeutic engagement.

• Conduct comprehensive psychosocial assessments to gain insight into clients' mental health concerns, strengths, and available resources.
• Provide individual, family, and group therapy utilizing evidence-based treatment modalities such as cognitive-behavioral therapy and solution-focused therapy.
• Develop and implement personalized treatment plans, outlining specific goals and interventions tailored to each client's unique needs.
• Collaborate with multidisciplinary teams, including psychiatrists and psychologists, to ensure integrated and coordinated care for clients.
• Provide crisis interventions, conduct risk assessments, and engage in safety planning as necessitated by clients' situations.
• Maintain accurate and timely documentation to adhere to ethical and legal standards, ensuring that client records reflect their treatment journey.
• Facilitate engaging group therapy sessions and psychoeducation classes, equipping participants with tools for coping and resilience.

• A Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.
• A minimum of two years of experience providing mental health services in outpatient or relevant settings.
• A strong understanding of evidence-based therapeutic modalities and best practices in mental health treatment.
• Exceptional interpersonal, communication, and assessment skills that foster trust and rapport with clients.

• Experience working in hospice or end-of-life care settings is a plus.
• Familiarity with telehealth platforms and remote therapy practices.
• Advanced training in specific therapeutic techniques such as trauma-informed care and grief counseling.
