As a remote healthcare provider, you will work from the comfort of your home while connecting with patients through our secure telehealth platform. This role allows for significant autonomy and flexibility in scheduling, enabling you to effectively manage your caseload while providing the highest level of care. Our hospice center focuses on providing supportive mental health care, particularly for individuals coping with the psychological impacts of chronic illness and end-of-life issues.

• Deliver evidence-based mental health treatment to a diverse patient population primarily through telehealth modalities, including both video and telephone sessions.
• Conduct comprehensive intake assessments to understand patient needs and formulate treatment plans that are tailored to individual circumstances.
• Facilitate individual, group, and family therapy sessions, adapting therapeutic techniques to best fit the unique dynamics and needs of each group.
• Administer mental health assessments including risk evaluations, and provide timely counseling, referral services, and intervention strategies as needed.
• Maintain meticulous and up-to-date documentation in compliance with Federal, State, and organizational guidelines, ensuring a seamless flow of patient care information.
• Engage in ongoing collaboration with interdisciplinary team members to ensure holistic and cohesive patient care strategies.

You will primarily work with individuals experiencing mental health issues related to chronic health conditions, providing essential support and guidance to enhance their emotional and psychological well-being. Your expertise will play a critical role in helping patients navigate the complexities of illness and improve their quality of life.

• A Master’s degree in Social Work from an accredited institution.
• An active and unrestricted LCSW license issued by the State of California.
• A minimum of three years of relevant experience in providing therapeutic services in a clinical setting.
• Proficient in using computers and telecommunication technologies, with prior experience in telemedicine considered a plus.
• Strong verbal and written communication skills to effectively convey information and build rapport with patients.

• Experience in hospice or palliative care settings is highly desirable, as it aligns closely with our patient population.
• Familiarity with evidence-based therapeutic approaches such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT).
• Background in crisis intervention and handling high-stress situations effectively.

We offer a competitive annual salary ranging from $120,000 to $130,000, commensurate with experience and qualifications.
