Key Responsibilities
  • Provide real-time support to the practitioner team in identifying and managing risk levels for clients, ensuring timely intervention when necessary.
  • Lead the escalation of concerns to emergency services, adhering to mandatory reporting guidelines when thresholds for reporting are met.
  • Regularly communicate with Clinical Leads regarding new safeguarding risks, participating actively in strategic mitigation efforts.
  • Stay up to date with safeguarding policies and procedures, ensuring they are integrated into day-to-day operations and addressing any challenges encountered.
  • Monitor and evaluate client outcomes, gathering feedback and insights to continuously develop and enhance the services provided.
Patient Population / Client Focus

In this position, you will primarily engage with patients in hospice care who may be facing end-of-life issues, including serious illness, grief, and psychological distress. The families of these patients will also be a significant focus, as you provide essential support and counseling to help them navigate this difficult journey. Your work will include addressing emotional, social, and logistical challenges while providing practical guidance and support.

Qualifications
  • Master's Degree in Social Work from an accredited institution.
  • Current and valid California Licensed Clinical Social Worker (LCSW) license is required.
  • A minimum of 2 yearsโ€™ experience in a behavioral health setting, preferably with a focus on hospice or palliative care.
Preferred Experience
  • Experience conducting psychosocial assessments and comprehensive case management evaluations, familiar with interview techniques and sensitive to issues such as suicide assessments.
  • Proficiency in mental health diagnostic criteria and familiarity with psychotherapeutic modalities.

Compensation and Benefits
