Work Environment

Key Responsibilities

• Act as a liaison between families and outpatient providers, ensuring that all parties are well-informed and engaged in the care process.

• Engage with insurance companies for pre-certification, facilitating the necessary documentation and information for client care and utilization management effectively.

• Participate actively in discharge and aftercare planning, collaborating with medical staff and clients to ensure smooth transitions and continuity of care.

• Provide therapeutic meal support, modeling a healthy relationship with food and addressing any emotional or psychological factors impacting clients’ eating habits.

• Deliver safe, empathetic, and affirming care tailored to the unique circumstances of each client, considering their intersecting identities and experiences.

Patient Population / Client Focus

You will primarily work with older adults facing a variety of challenges, including chronic illnesses, mental health issues, and end-of-life transitions. Your role is critical in ensuring these clients receive the supportive care necessary to navigate their experiences with dignity and respect, thus enhancing their quality of life.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.

• Must hold a valid LCSW license in the state of Kansas.

• Experience working with older adults, demonstrating an understanding of their unique challenges and needs.

• Strong clinical and diagnostic skills are essential to effectively assess and support clients.

• Excellent communication and interpersonal skills are a must for effectively engaging with clients, families, and other stakeholders.

• The ability to work independently and as part of a cohesive team is crucial for success in this role.

Preferred Experience

Additional experience in hospice care or palliative support, as well as familiarity with community resources in Kansas for older adults, is highly valued. Candidates who demonstrate cultural competence and the ability to navigate varying psychosocial dynamics will be preferred.

Compensation and Benefits
