Wellmore Health Center is seeking a dedicated and compassionate Full-Time Licensed Clinical Social Worker (LCSW) who is committed to providing exceptional mental health support to clients in our Hospice Center. Key Responsibilities
  • Act as a liaison between families and outpatient providers, ensuring that all parties are well-informed and engaged in the care process.

  • Engage with insurance companies for pre-certification, facilitating the necessary documentation and information for client care and utilization management effectively.

  • Participate actively in discharge and aftercare planning, collaborating with medical staff and clients to ensure smooth transitions and continuity of care.

  • Provide therapeutic meal support, modeling a healthy relationship with food and addressing any emotional or psychological factors impacting clients’ eating habits.

  • Deliver safe, empathetic, and affirming care tailored to the unique circumstances of each client, considering their intersecting identities and experiences.

You will primarily work with older adults facing a variety of challenges, including chronic illnesses, mental health issues, and end-of-life transitions. Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.

  • Must hold a valid LCSW license in the state of Kansas.
