Key Responsibilities
  • Act as a liaison between families and outpatient providers, ensuring cohesive communication and supporting the continuum of care.
  • Engage with insurance companies to facilitate pre-certification requirements and manage utilization effectively, advocating for patient care needs.
  • Participate in discharge and aftercare planning processes to provide seamless transitions for patients leaving care facilities.
  • Provide therapeutic meal support, which includes modeling healthy relationships with food and guiding clients in creating positive eating habits.
  • Deliver safe, empathetic, and affirming care that respects each client's intersecting identities, ensuring each person feels valued and understood.
Patient Population / Client Focus

Your primary focus will be on older adults, a demographic that often faces complex social and emotional challenges. You will provide tailored support for this population, addressing issues such as grief, loss, adjustments to aging, and chronic illnesses. Your work will contribute to improved client outcomes through targeted interventions and a compassionate approach, recognizing the unique needs this population presents.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • A valid LCSW license in the state of Maryland is required.
  • Experience working with older adults and an understanding of the specific challenges they face.
  • Proficient clinical and diagnostic skills, with demonstrated experience crafting individual treatment plans.
  • Excellent communication and interpersonal skills to effectively build relationships with clients, families, and multidisciplinary teams.
  • Ability to work independently while also thriving in a collaborative team environment.
Preferred Experience
  • Experience in hospice settings or palliative care is strongly preferred.
  • Familiarity with telehealth platforms and electronic health records is advantageous.
  • Skills in crisis intervention and strong problem-solving abilities are highly valued.
Compensation and Benefits
