Key Responsibilities
  • Serve as a vital liaison between clients, families, and outpatient providers to facilitate seamless care coordination.
  • Interact with insurance companies to manage pre-certification and utilization processes, ensuring that clients receive timely and appropriate benefits.
  • Participate actively in discharge planning and aftercare coordination to support clients’ transitions and ongoing recovery.
  • Provide therapeutic meal support by guiding clients in developing a healthy relationship with food, demonstrating best practices in nutrition and self-care.
  • Deliver empathetic and affirming care tailored to the unique circumstances and intersecting identities of each client, enhancing their overall therapeutic experience.
Patient Population / Client Focus

The licensed clinical social worker will primarily serve a diverse community of clients encompassing various ages, with a particular focus on older adults.
