Work Environment

As a Licensed Clinical Social Worker, you will work remotely in a flexible environment that supports work-life balance. Your primary work facility will be our Community Health Clinic, which serves as a hub for a range of mental health services. Key Responsibilities

• Serve as a vital liaison between clients, families, and outpatient providers to facilitate seamless care coordination.
• Interact with insurance companies to manage pre-certification and utilization processes, ensuring that clients receive timely and appropriate benefits.
• Participate actively in discharge planning and aftercare coordination to support clients’ transitions and ongoing recovery.
• Provide therapeutic meal support by guiding clients in developing a healthy relationship with food, demonstrating best practices in nutrition and self-care.
• Deliver empathetic and affirming care tailored to the unique circumstances and intersecting identities of each client, enhancing their overall therapeutic experience.

Patient Population / Client Focus

The licensed clinical social worker will primarily serve a diverse community of clients encompassing various ages, with a particular focus on older adults. This demographic requires specialized attention and support due to their unique social, psychological, and physical challenges. Our clinicians play a critical role in addressing mental health issues, adjusting treatment approaches to align with the clients' individual needs, and promoting resilience among vulnerable populations.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current and valid LCSW license in the state of Colorado.
• Demonstrated experience working with older adult populations, understanding the complexities involved in their care.
• Strong clinical evaluation and diagnostic skills, enabling effective identification of mental health conditions.
• Excellent verbal and written communication abilities, enhancing collaboration within the team and with clients.
• A proven capacity to work independently while being an active member of a multidisciplinary team.

Preferred Experience

• Experience in a community health setting, particularly in remote care models.
• Familiarity with various therapeutic modalities and interventions suitable for seniors.
• Proficiency in utilizing electronic health record (EHR) systems and case management software.

Compensation and Benefits

The salary for this position ranges from $90,000 to $105,000 annually, commensurate with experience and qualifications.
