Key Responsibilities
  • Serve as a vital liaison between families and outpatient providers, ensuring effective communication and continuity of care.
  • Interact with insurance companies to manage pre-certification and utilization management processes, advocating for our clients' health needs.
  • Engage in discharge planning and aftercare strategy development, supporting clients in their transitions from care.
  • Provide therapeutic meal support, guiding clients through modeling healthy relationships with food and nutrition.
  • Deliver safe, empathetic, and affirming care tailored to the unique circumstances of each client, with consideration for their intersecting identities.
Patient Population / Client Focus

This role primarily serves a diverse clientele that includes individuals facing various mental health challenges, particularly older adults. By understanding the specific needs of this population, you will develop and implement personalized care plans, ensuring each patient receives the specialized support they require. Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • A valid Licensed Clinical Social Worker (LCSW) license in the state of Texas is required.
  • Demonstrated experience working with older adults, along with strong clinical assessment and diagnostic skills.
  • Exceptional interpersonal and communication skills, enabling effective collaboration with clients and colleagues alike.

Compensation and Benefits
