• Conduct thorough assessments and screenings to capture each patient's mental health history, ensuring accurate diagnosis and developing tailored treatment plans that suit individual needs.
• Provide short-term individual counseling sessions, applying crisis intervention techniques to support patients dealing with acute psychological distress.
• Facilitate therapy groups and workshops that focus on relevant topics such as stress management, coping strategies, and personal development, promoting peer support among participants.
• Collaborate closely with a multidisciplinary team, including physicians, nurses, and other mental health professionals, to coordinate holistic care and referral processes.
• Engage in community outreach initiatives, such as mental health awareness campaigns and wellness workshops, to promote mental health education and access to services.
• Maintain accurate, up-to-date, and confidential patient records in compliance with HIPAA regulations and ethical standards of practice.

You will engage with a diverse range of patients seeking support for various mental health challenges, including anxiety, depression, trauma, and stress-related disorders. Understanding the unique cultural and socioeconomic factors affecting our patients in the Pomona community is essential in delivering empathetic and effective care. Your contribution will directly impact their therapeutic journey, helping them navigate difficult times and empowering them toward recovery.

• Must hold an active state license as a Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Psychologist.
• A Bachelor's degree in Psychology, Counseling, Social Work, or a related field is required; a Master’s or Doctoral degree is preferred.
• Demonstrated knowledge of evidence-based practices and brief therapy models, particularly suited for a remote counseling environment.
• Excellent communication and interpersonal skills with a strong ability to engage with patients from diverse backgrounds and collaborate effectively with a variety of stakeholders.
• Proficiency in using telehealth platforms and other digital tools to deliver services effectively.

• Experience working in a community health setting or a similar clinical environment is highly desirable.
• Previous involvement in outreach initiatives, wellness events, or mental health education programs is a plus.
• Familiarity with therapeutic modalities and approaches tailored to the unique challenges faced by individuals within a community health context.

• Annual salary range of $115,000 - $130,000, commensurate with experience and qualifications.
