• Conduct thorough clinical assessments to evaluate the emotional, psychological, and social needs of patients and their families.

• Develop, implement, and review personalized treatment plans that incorporate evidence-based, solution-focused therapeutic interventions.

• Maintain clinical documentation and timely record-keeping to ensure compliance with healthcare regulations and to facilitate effective team communication.

• Collaborate actively with a multidisciplinary team of healthcare professionals, including physicians, nurses, and chaplains, to provide comprehensive patient care.

• Provide education and emotional support to patients and their families, guiding them through the challenges associated with end-of-life decisions and bereavement.

• Stay informed about the latest developments in mental health care and therapeutic interventions to enhance the effectiveness of your practice.

In this role, you will primarily work with patients in hospice care—individuals facing terminal illnesses and their families. You will employ empathetic listening, support, and therapeutic techniques to address emotional distress and promote quality of life during challenging times. Your work will play a crucial part in helping families navigate grief, understand the dying process, and find peace.

• A Master’s degree in Social Work from an accredited institution is required.

• A valid LCSW or LCSW-R license in the state of New York is mandatory.

• Proficiency in solution-based psychotherapy approaches is essential, supported by demonstrated experience in the field.

• Exceptional communication and interpersonal skills to effectively convey empathy and understanding to patients and families.

We expect candidates to have a minimum of two years of experience in social work, ideally within a clinical or hospice care setting. Familiarity with end-of-life care principles and experience working within a multidisciplinary healthcare team will be highly advantageous.

• The annual salary for this position ranges from $80,000 to $100,000, commensurate with experience and qualifications.
