As a remote employee working from the comfort of your own home, you will have the resources and support needed to effectively meet clients' needs. Our hospice center connects you with a team focused on holistic care and compassionate support for individuals with terminal illnesses. You will utilize state-of-the-art telecommunication tools that facilitate seamless virtual therapy sessions.

• Conduct individual and group therapy sessions through secure video conferencing platforms, ensuring a private and empathetic setting.
• Develop comprehensive treatment plans tailored to the unique needs of each client, incorporating evidence-based practices to enhance therapeutic outcomes.
• Collaborate with interdisciplinary team members, including nurses and physicians, to provide integrated care and address the multifaceted needs of patients.
• Utilize electronic health record systems to maintain accurate and up-to-date client records, including progress notes and treatment modifications.
• Engage in ongoing professional development to stay abreast of the latest therapeutic techniques and regulatory requirements.

You will be working with a diverse population of clients facing terminal illnesses and their families, offering emotional support and coping strategies during one of the most challenging times in their lives. Your expertise in palliative care will be crucial in helping them navigate their thoughts and emotions surrounding end-of-life decisions, grief, and loss.

• Current and valid LCSW licensure.
• A Bachelor’s degree in Social Work from an accredited institution, with a focus on clinical practice.
• Strong interpersonal skills with the capability to establish rapport with clients from varied backgrounds.
• Proficient in using telehealth platforms and familiar with healthcare technology.
• Demonstrated ability to work independently and manage multiple cases effectively.

• At least two (2) years of experience in a clinical social work setting, preferably with a focus on hospice or palliative care.
• Experience providing therapy through telehealth platforms and familiarity with digital record-keeping systems.
• Expertise in working with patients and families from diverse cultural backgrounds.
