Social Work Director

WeCare Centers atHeritage Care Center is actively seeking a motivated Social Work Director for our community in Pittsburgh, PA.

Key Responsibilities

  • Reviews and follows up on any concerns for the residents and staff with all incident reports.

  • Works with caregivers of the nursing unit and all departments to provide the residents with a fulfilling living experience.

  • Keeps supervisor informed of unusual occurrences that relate to the health and safety of residents.

  • Oversees care plans, MDS, and quality improvement processes.

  • Keeps Admin fully informed of stats, progress and achievements.

  • Must have the ability to deal tactfully with personnel, residents, family members, visitors and the general public.

  • Must be able and willing to work well with inter-disciplinary teams.

  • Any other duties as assigned.

Skills, Knowledge and Expertise

  • Bachelor's Degree in Social work or Human Services.

  • Previous experience in geriatrics or long-term care.
