Cedar Haven Healthcare Center is seeking a dedicated Social Services individual to join the Social Services Department within our nursing home. This role serves as a liaison and advocate for residentsโ€™ social interests and is responsible for planning, organizing, implementing, and evaluating the Social Services Dept

Position Overview

This individual ensures that residentsโ€™ medically related social and emotional needs are identified and addressed through comprehensive assessments, individualized care planning, and ongoing support. This position plays a vital role in resident adjustment, family communication, crisis intervention, and regulatory compliance.

Key Responsibilities
  • Develop comprehensive social assessments and individualized care plans that identify medically related social and emotional needs, establish realistic goals, and outline specific interventions

  • Actively participate in pre-admission and admission processes

  • Serve as a compassionate resource to residents and family members during periods of adjustment, crisis, or special need

  • Document residentsโ€™ behavioral and psychosocial status accurately and timely

  • Provide individual and group support services to residents and families

  • Establish, coordinate, and support a volunteer program designed to meet specific social service needs

  • Orient residents and families to the nursing center and assist with transition and adjustment to the new environment

  • Ensure compliance with federal, state, and local regulations governing social services in long-term care settings

Qualifications
  • Minimum requirement: Social Work experience or a related field

  • Previous long-term care (LTC) experience desirable

  • Past supervisory experience preferred

  • Strong knowledge of social service practices, procedures, laws, regulations, and guidelines governing long-term care

  • Excellent communication, organizational, and interpersonal skills

  • Ability to work independently while managing multiple priorities effectively
