• Lead psychoeducational therapy groups and conduct one-on-one therapy sessions, focusing on emotional well-being and coping strategies for hospice patients.
• Perform thorough clinical assessments upon patient admission to establish individualized care plans that address both psychological and social needs.
• Collaborate effectively with the Utilization Review Department and Business Department to manage authorizations and payments related to services provided.
• Initiate discharge planning on the day of admission, ensuring a proactive approach to patient transitions and care continuity.
• Conduct discharge sessions to review coping skills, transition plans, and relevant resources, ensuring patients and families are fully informed.
• Communicate with referral sources and outpatient providers at admission, midpoint, and discharge to facilitate optimal patient care.
• Document all services and interactions in a timely manner, adhering to all regulatory and organizational standards for compliance and quality care.

In this role, you will engage with patients facing serious illness and their families, providing them with tailored support that encompasses both psychological and social dimensions of care. You will work closely with individuals and their loved ones, helping them navigate complex emotional responses associated with end-of-life issues, and providing necessary resources to enhance their quality of life.

• Master’s degree in Social Work from an accredited institution.
• Current and valid license as a Clinical Social Worker (LCSW) in the state of California.
• Strong written and verbal communication skills for effective interaction with patients, families, and interdisciplinary teams.
• Proven ability to establish and maintain appropriate professional boundaries with clients.
• Ability to manage documentation processes efficiently, completing all required paperwork accurately and in a timely fashion.

• Experience in hospice or palliative care settings is highly desirable.
• Previous case management experience or familiarity with interdisciplinary care models.
• Knowledge of community resources and support services for patients with terminal illnesses.
• Proficiency in electronic health record systems for effective documentation and coordination of care.

The position offers a competitive salary range of USD 120,000 to 130,000 per year, commensurate with experience and caseload.
