• Conduct thorough assessments by interviewing clients to evaluate their emotional, social, and financial needs, ensuring a comprehensive understanding of their unique circumstances.
• Develop and implement tailored support plans that address each client's specific challenges and goals, facilitating their journey toward recovery and improved quality of life.
• Regularly monitor clients' progress on their goals, adjusting treatment plans based on individual responses and outcomes.
• Provide crisis intervention when necessary, helping clients manage and navigate emergencies effectively.
• Coordinate with external agencies and community resources to ensure clients receive holistic support, offering referrals to other services when required.
• Maintain accurate and detailed records of client interactions, treatment plans, and progress notes to uphold the highest standards of accountability and professionalism.
• Participate in case conferences, collaborating with multidisciplinary team members to discuss client progress and treatment adjustments.
• Attend court proceedings as needed to provide evidence and advocate for client needs.

You will primarily work with clients facing end-of-life challenges within a hospice setting, including individuals with terminal illnesses, their families, and caregivers. This position requires sensitivity and expertise in managing emotional distress, grief, and the complexities of navigating healthcare systems during difficult times.

• Master's degree in Social Work (MSW) from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory to ensure compliance with professional standards.
• Proven clinical experience delivering social work services, counseling, and crisis intervention, particularly in hospice or related settings.
• Excellent communication skills, both verbal and written, with an emphasis on active listening and empathic engagement.

We offer a competitive salary ranging from USD 75,000 to 90,000 annually, depending on experience and qualifications.
