Provide Dialysis Care In Patients' Homes - One Patient At A Time

This is a field-based, patient-facing role serving patients within the Broward County, FL market, ideal for individuals who bring empathy, emotional intelligence, and follow-through to their work.

Key Responsibilities

  • Reinforce dialysis education and provide patient and caregiver instruction using approved materials
  • Confirm comprehension using teach-back and observation
  • Deliver compassionate, patient-centered support in patient homes
  • Identify and escalate patient concerns or changes to the supervising RN
  • Travel within the Broward County territory to support patients in their homes
  • Assist with dialysis equipment setup, breakdown, and basic troubleshooting within scope and prior training
  • Perform blood draws and specimen collection per protocol
  • Document patient interactions and education in the EMR
  • Maintain compliance with all policies, procedures, and care standardsย 
Qualifications

Required
  • Minimum 1.5 years of prior dialysis experience as a Patient Care Technician supporting home dialysis (HHD and/or PD)
  • Documented patient education experience (EMR education notes, care plans, modality education, onboarding, or training documentation)
  • CCHT or CHT certification
  • BLS certification
  • Completion of structured dialysis training through DaVita, Fresenius (FMC), or a comparable dialysis organization
  • High School Diploma or GED
  • Valid Florida Driverโ€™s License
  • Must have access to a reliable, insured vehicle for daily travel between patient homes throughout the workday
  • Experience using Electronic Medical Records (EMR) systems

  • Bilingual (English/Spanish or English/Creole)
