As a Licensed Professional Counselor, your primary duties will include:

• Collaborating with primary care providers to identify and manage mental health and psychosocial issues, ensuring a comprehensive treatment approach.
• Conducting thorough patient intakes, focusing on diagnostic assessments and functional evaluations to develop tailored treatment goals.
• Providing short-term, focused interventions to patients in need of mental health services and ensuring their active participation in the treatment process.
• Recommending referrals to specialty mental health services when appropriate, based on your assessments and the individual needs of patients.
• Tracking patient progress through documentation of treatment plans, outcomes, and follow-up strategies, maintaining clear communication with primary care providers.
• Assisting in developing relapse prevention plans for clients, helping them acquire strategies for ongoing stability and recovery.
• Offering educational resources and self-management skills to empower patients to take charge of their health and well-being.
• Consulting with clinic management and team members regarding best practices and program evaluations to continuously enhance service delivery.

You will work with a diverse patient population, including individuals facing various mental health challenges, adolescents, and families within the community. Understanding the personal and social impact of mental health issues is crucial as you tailor your approach to each patient's unique circumstances. Your role will directly influence the quality of life for those you serve, making empathy and understanding key components of your practice.

To thrive in this role, candidates must possess the following qualifications:

• A Master’s Degree in Clinical Counseling or a related field.
• A current Licensed Professional Counselor (LPC) license in the State of Oregon.
• Comprehensive knowledge of mental health assessment tools, treatment planning methods, and psychotherapeutic techniques.
• Ability to understand and apply behavioral therapy principles, as well as a solid grasp of DSM-IV diagnoses and developmental psychopathology.
• Excellent interpersonal and communication skills, enabling you to build effective relationships with team members and clients alike.
• Proficiency in maintaining accurate clinical records and preparing reports that meet regulatory standards.

While not mandatory, preferred qualifications include:

• Proficiency in both English and Spanish, enabling you to effectively communicate with a broader range of patients.
• Experience working in community health settings or Federally Qualified Health Centers (FQHCs).
• Familiarity with coordination of care across interdisciplinary teams and knowledge of community resource networks.
