This position offers the flexibility of a remote work setting, allowing our LCSWs to engage with clients from the comfort of their home while maintaining the standard of care expected within our hospice environments. • Conduct regular wellness checks via telehealth platforms to monitor client progress and adjust care plans effectively.
• Evaluate the psychological and social needs of clients and their families, ensuring all aspects are addressed in their care journey.
• Engage proactively with community resources, providing clients with appropriate referrals and support services tailored to their needs.
• Collaborate with clinical teams to provide additional support when required, ensuring a seamless transition of care during team absences.
• Maintain accurate and timely documentation of services provided, including treatment plans and progress notes, in adherence to organizational policies and licensure standards.
• Communicate effectively with the Utilization Review and Business Departments regarding authorization and billing processes for client services.

In this role, you will primarily work with terminally ill patients and their families, navigating the complex emotional landscape that accompanies end-of-life scenarios. Your expertise will help clients process grief, fears, and fostering acceptance while also providing their families with the necessary support and resources to cope with loss effectively.

• A Master’s degree in Social Work from an accredited institution.
• An active license as a Clinical Social Worker (LCSW) in the state of California.
• Proven competency in both written and verbal communication skills.
• Ability to establish appropriate professional boundaries while fostering trust with clients.
• Strong organizational skills to manage documentation and client interactions efficiently.

• Experience working within hospice or end-of-life care settings is highly desirable.
• Familiarity with telehealth tools and experience providing remote counseling services.
• Background in community resource management and case management.

• Competitive salary offering between $120,000 and $130,000 yearly, contingent on experience and caseload.
