• Conduct thorough clinical assessments of patients to understand their psychosocial needs, guiding the formulation of individualized treatment plans.
• Utilize evidence-based solution-focused therapeutic interventions to support patients and families in navigating challenges related to serious illness.
• Keep accurate and timely clinical records that comply with healthcare regulations and organizational standards, ensuring all data is securely maintained.
• Work collaboratively with a dedicated team of healthcare professionals, including nurses, doctors, and chaplains, to provide holistic care.
• Offer psychoeducation and emotional support to patients and their families, assisting them in coping with the complexities of end-of-life care.
• Stay informed about the latest mental health research and practices to continuously enhance the support provided to clients.

Our LCSWs will focus on a diverse patient population, primarily individuals facing terminal illnesses and their families. This role requires sensitivity and adaptability, as social workers will assist patients in managing emotional, social, and practical issues arising from their health circumstances. By employing tailored interventions, you will empower families to navigate their unique journeys with grace and understanding.

• A Bachelor's degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in the state of New York is essential.
• Demonstrated knowledge and experience in solution-focused psychotherapy techniques.
• Strong communication and problem-solving skills, with the ability to establish rapport quickly and effectively with patients and families.

• A minimum of 2 years of clinical social work experience is required, ideally in a healthcare setting such as hospice or palliative care.
• Previous work with diverse populations and experience in crisis intervention will be highly valued.
