Heart Mind & Soul Center for Wellbeing is seeking a dedicated and compassionate full-time Licensed Independent Clinical Social Worker (LICSW) to join our dynamic team. This position is unique as it is fully remote, allowing for flexible work arrangements while serving a crucial role within our hospice center. While working from home, you will have the technological support to conduct sessions, participate in virtual meetings, and remain fully engaged with patients and team dynamics.

• Conduct thorough psychosocial assessments to evaluate patients' mental health status, strengths, and needs, utilizing telehealth tools and methodologies.
• Deliver evidence-based therapeutic interventions including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) tailored to individual needs.
• Collaborate with a multidisciplinary team, including psychiatrists and nursing staff, to coordinate care plans that address each client’s unique circumstances.
• Facilitate individual, group, and family therapy sessions, employing diverse evidence-based modalities to promote healing and recovery.
• Generate comprehensive treatment plans in cooperation with therapy teams, integrating patient goals, preferences, and therapeutic needs.
• Advocate for patients and their families, ensuring they understand their diagnoses, treatment options, and required resources.
• Lead psychoeducation sessions and therapeutic groups to enhance patient and family support within the mental health system.
• Assist in discharge planning meetings to ensure a smooth transition for patients within the continuum of care.

Our hospice center serves a diverse range of clients, including individuals facing end-of-life challenges and their families. You will work with patients dealing with complex emotional and mental health needs, ensuring that they have access to quality psychosocial support. Your efforts directly contribute to improving the quality of life for these individuals, providing them with the tools they need to navigate emotional distress and facilitating their path to emotional and mental well-being.

• A Master’s degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) is essential.
• Proficiency in mental health disorders, treatment modalities, and crisis intervention techniques.
• Experience in a multidisciplinary team setting, demonstrating collaborative skills and effective communication.

• Experience working in hospice or end-of-life care environments is highly preferred.
• Familiarity with remote therapeutic tools and telehealth platforms.
• Previous experience in providing psychoeducation or therapeutic groups is a plus.

The salary for this position ranges from USD 80,000 to 100,000 annually, with additional performance-based bonuses. In addition to competitive pay, we offer:
